Please complete your intake forms at least 24 hours prior to your first massage session.
You only have to fill out the intake forms once every 2 years, so please take the time to fill them out thoroughly and with as much information as possible to make your intake process during the appointment smooth and efficient.
Please keep in mind if you do not fill out your intake forms more than 24 hours prior to your appointment, that the time it takes for the therapist to thoroughly review your intake forms may cut into your session time. This is non negotiable as it is important for your health and wellbeing that your massage therapist fully understand your health history and confirms that massage is safe for you to receive.
Refusal to complete your intake form(s) will result in the cancelation of your appointment and the applicable fee’s will be charged.
